Information we collect

We collect information you voluntarily provide through our contact and Savings Pass request forms, such as your name, organization, email address, telephone number, state, provider or organization type, estimated application count, and message.

Our hosting provider may also process standard technical information needed to deliver and protect the website, including IP address, browser information, device information, timestamps, and security logs.

We use Google Analytics and Google Ads measurement tools to understand visits, advertising performance, and program actions such as Savings Pass eligibility requests, checkout starts, and confirmed pass purchases. These tools may use cookies or similar identifiers. We do not send names, email addresses, EINs, Savings Pass codes, Social Security numbers, patient information, protected health information, uploaded documents, or form-message contents to Google Analytics or Google Ads.

How we use information

• Respond to questions and requests.
• Review eligibility, process payment, issue and manage Savings Passes, prevent duplicate use, and maintain program records.
• Operate, secure, troubleshoot, and improve the website.
• Measure program reach and advertising effectiveness without sending credentialing or sensitive information to advertising platforms.
• Meet legal, accounting, security, and recordkeeping obligations.

Service providers and optional provider handoff

The website and forms are hosted by Netlify. Stripe processes Savings Pass payments. Resource articles may be delivered through LinkArtemis. Google provides website analytics and advertising measurement. After NJ Impact issues a pass, you may elect to continue to the participating credentialing provider. Information you submit directly to that provider is governed by the provider's own notices and agreements. NJ Impact does not automatically transfer your sensitive credentialing documents through this public website and does not sell personal information.

Retention and choices

We retain submitted information only as reasonably needed for the purposes above, recordkeeping, security, and legal obligations. You may ask to review, correct, or delete information by emailing info@njimpact.org. Some records may be retained when required by law or legitimate operational needs.

Security and children

We use reasonable safeguards, but no online transmission or storage method is guaranteed secure. This website and its forms are intended for adults acting for themselves or an organization and are not directed to children under 13.

Changes

We may update this notice as the program or website changes. The date at the top identifies the current version.
